High cholesterol. Pre-diabetic. On the verge of fatty liver disease. Karandeep walked into a routine check-up feeling fine, and walked out with three warnings he could not ignore.

Karandeep Singh is an aerospace engineer, 42 years old, in a job built on very early starts, relentless attention to detail and long stretches of high pressure. Across lockdown the weight crept on, old injuries lingered, and the lifestyle that came with the career slowly took its toll. He was vegetarian and reasonably active, which made the blood results all the more of a shock.

Karandeep had never worked with a personal trainer, and walking into a public gym was the part that worried him most. Being an engineer, he wanted a plan built on evidence rather than guesswork, and a coach who understood his vegetarian diet and his background. He sat on the fence for a long time. Then he took the plunge.

I was very nervous about getting started. I had never worked with a PT before, and never trained in a public gym. But in the first couple of weeks the weight was coming off quickly, and that was a big motivator. The thing I was nervous about just was not there. I felt comfortable straight away.

Built around data, not guesswork

Karandeep is a numbers man, so the coaching was built on numbers. Every morning he logged his weight, his step count and whether he had eaten out, into a daily tracker that turned his progress into live, everyday data. The plan was shaped around his nutrition, his shifts and his social life, so it slotted into his routine instead of fighting it, and the accountability never let up.

Every morning I put in my weight, my step count, whether I had meals out. It was real, live, everyday data we could use to see why things were working. If I forgot to fill it in, Tanvir was on it straight away. It kept me honest, and the progress was right there in front of me, not guesswork.

As a vegetarian, I did not want a trainer who would tell me to eat meat, or that you cannot get enough protein without it. Tanvir came from a similar background, he understood our culture and our diet. For me, that was what set him apart.

What those results actually mean

High cholesterol usually has no symptoms, which is exactly what makes it easy to ignore. Over time, excess cholesterol builds up inside the artery walls, narrowing them and raising the risk of a heart attack or stroke. It is picked up through a simple blood test. As a general guide in the UK, a total cholesterol below 5 mmol/L is considered healthy, though your GP will read your full panel in context.

Pre-diabetes means blood sugar sits higher than normal but not yet in the diabetic range. It is a warning sign, not a diagnosis. Blood sugar is measured by HbA1c: below 42 mmol/mol is normal, 42 to 47 is the pre-diabetic range, and 48 or above indicates diabetes. Left unchecked it can progress to type 2 diabetes, with longer-term risks to the heart, kidneys, eyes and nerves. The encouraging part is that at this stage it is very often reversible through weight loss and lifestyle change.

Fatty liver means fat has started to build up in the liver, usually linked to excess weight, diet and inactivity. In the early stages there are often no symptoms at all. Left to progress, it can lead to inflammation, scarring and lasting liver damage. Caught early, it is one of the most reversible of the three, which is exactly why early blood tests matter.

If any of this sounds familiar, the most useful thing you can do is book a blood test with your GP. Knowing your numbers is the first step to changing them.

Six months on, the picture had changed completely. Karandeep dropped 8kg, from 75.4kg to 67.3kg, and his body fat fell from 16.7 percent to 12 percent. He took 12cm off his waist, down from 89cm to 77cm. Most importantly, when he went back to his GP, his blood results came back in the healthy range across the board. The cholesterol, the blood sugar, the liver markers, all back where they should be.

Tanvir met and exceeded my expectations. My blood results all came back in the green, and my clothes fit so differently I need a new wardrobe, which is a nice problem to have. For anyone sitting on the fence, do it. I took the plunge, and it was probably the best decision I have ever made.

A note from your coach
